INSTRUCTIONS FOR COMPLETING THE EPOCH BUSINESS ANALYSIS QUESTIONNAIRE There are 100 numbered questions. Indicate your answer to each one of them by choosing one of the three options provided for each answer. + (Plus) means I AGREE, YES, or MOSTLY YES. M (Middle) means UNCERTAIN, MAYBE, NEITHER YES NOR NO. - (Minus) means I DISAGREE, NO, or MOSTLY NO. Answer the questions based on the main product or service that you deliver. “Customers” includes “clients”, “patients”, and so on as appropriate. Each question must be answered in order for the questionnaire to be valid. If an answer is left blank, you will get an error message when you try to send the form. In that case, you will need to go back and answer any unanswered questions. So answer each question as best as you can.
